Texas IPS’s Post‑ICU Outpatient Clinic offers structured, compassionate follow‑up beginning within weeks of hospital discharge. In a multidisciplinary format, our team—including physicians, respiratory therapists, and care navigators—helps patients manage lingering symptoms, regain function, and reduce the risk of complications.

Nurse-led transitional care programs significantly reduce 30-day readmission rates, emergency department visits, and improve quality of life.

Our program provides:

  • Comprehensive evaluation of physical symptoms, mood, cognition, and breathing capacity
  • Medication review & reconciliation to prevent errors and side effects
  • Rehabilitation planning, including referrals to pulmonary or physical therapy
  • Education & self‑management coaching, including fatigue coping and breathing techniques
  • Scheduled follow‑ups—in‑clinic or via telehealth—to monitor progress and intervene early if needed
